Commissioner of Oath and Notary Public

Notarization / Legalization / Certified true copy

A certified true copy is a copy of a document that is stamped and signed as a true copy of the original by a person who is allowed to take declarations. lt does not certify that the primary document is genuine, only that it is a true copy of the primary document.

Consent to Travel Document

Drafting and notarization of consent to travel document is required because of increasing instances of child abduction in custody cases, and a growing number of children who are the victims of trafficking or pornography, an immigration officer, airline,or travel company may ask you to provide some form of letter of consent if your child is traveling internationally with only one parent or with another adult, such as a grandparent, aunt, uncle etc.

Valid Government-Issued Picture ID Please bring in a valid Government-issued picture ID with you. Please make sure the ID is not expired. The identification document must also indicate your signature. Acceptable IDs include Driver’s Licence, Passport. Please note health card is not a valid picture ID in Ontario.

Do NOT Sign or Date Documents
Please do not sign or date the documents prior to coming to your appointment. Once you come to the office, Notary Public will identify you and witness your signatures.

Complete all documents
Prior to your appointment, please make sure you complete the documents except for signatures and dates.

Certified Documents
Please make sure that you bring the original document as Notary Public must view the original in order to certify the documents.
